A readable log does not need every possible health detail. For most manual logging
workflows, the durable facts are the glucose value, the unit (mg/dL or mmol/L),
the date and time, and one timing label such as fasting, before meal, after meal, or
bedtime. Optional notes are useful when they explain context your clinician asked you
to watch, such as illness, medication timing, exercise, or a specific meal.
The American Diabetes Association notes that logging blood glucose results helps your care team see how your body responds to the care plan. It also lists common targets for many nonpregnant adults with diabetes as 80-130 mg/dL before meals and less than 180 mg/dL 1-2 hours after the beginning of a meal. Treat those as reference values, not personal instructions.
How to use the sheet
Print one sheet per week. Write the unit next to every value. If you are logging after meals, write down whether the reading is one hour or two hours after the start of the meal, because those are different review points. Bring the filled sheet to appointments only if your clinician wants a written log or you prefer paper.
For pregnancy-specific targets, do not use a generic internet chart as your plan. The ACOG gestational diabetes FAQ tells patients to keep a record of blood sugar levels and bring it to prenatal visits. NICE lists pregnancy targets below 5.3 mmol/L (about 95 mg/dL) fasting, below 7.8 mmol/L (about 140 mg/dL) 1 hour after meals, or below 6.4 mmol/L (about 115 mg/dL) 2 hours after meals when achievable without problematic hypoglycemia. Your obstetric care team may use different timing, units, or target numbers based on your pregnancy and local guidance.
When paper stops working
Paper is often the easiest way to start. It becomes harder when you need to filter fasting readings, compare before-and-after meal pairs, or share a clean summary before an appointment. That is where an app can help without changing the basic habit.
